Reporting Safety Hazards

There are four ways to report a health or safety hazard:

1. If you believe there is an immediate danger to the health or safety of yourself or others call Physical Plant at 7351 from a campus telephone or (574) 535-7351 from a cell phone. After hours, call Campus Security at 7599 or (574) 535-7599 from a cell phone.
2. If there is no immediate danger to the health or safety of yourself or others, but you believe there is an ongoing or potential hazard that may need attention, contact your supervisor.
3. If the problem cannot be remedied by a supervisor, or a supervisor is not available, contact Human Resources at (574) 535-7707.
4. If talking with your supervisor or a representative from Human Resources is not an option in reporting a workplace safety concern, the Campus Conduct Hotline is a toll-free number you can call to discuss your concerns about unsafe practices or conditions. The Campus Conduct Hotline is available at any time of day or night. No call-tracing or recording devices are used. If you wish, you may remain completely anonymous. Call (866) 943-5787.
