Reporting Accidents and Injuries
Goshen College’s Commitment to Health and Safety
Goshen College values the health and safety of all individuals engaging in college programs or activities on college properties. While we strive to maintain safe conditions and practices, we recognize that from time-to-time an individual may be injured while participating in campus activities or on college properties. It is important for everyone to understand the requirements and steps for reporting injuries and obtaining support. This information applies to student and campus visitors.

Reporting Accident and Injury
To report an accident or injury:
- If the injury is life-threatening or extremely serious, call 911. Afterward, call Campus Safety (574) 535-7599.
- All students must notify Campus Safety (574) 535-7599 immediately in the event of an accident or injury that occurs as a result of their participation in college activities or on college properties.
- Complete the Goshen College Injury and Illness Incident Report Form within 24 hours of the time of the injury or accident. When completed, send the form to Campus Safety.
- NOTE: Student-employees and supervisors of student-employees: If a student-employee is injured as a result of their employment duties, the student-employee’s supervisor should:
- Notify Human Resources immediately at (574) 535-7111 or (574) 312-5011. Evenings or weekends: Contact Campus Safety at (574) 535-7599. They will notify HR. Notifications of injury that occur at Merry Lea should follow a different contact chain.
- Complete the Employee Injury and Illness Incident Report Form within 24 hours of the time of the injury or accident. When completed, send the form to Human Resources.
Unless the accident is life-threatening or serious enough to require emergency medical services, the college must be notified immediately of any injuries related to college property or activities.
Reporting Safety Hazards
To report a health or safety hazard:
- If you believe there is an immediate danger to the health or safety of yourself or others, contact Campus Security through Omnilert, call at 7599 from a campus telephone, or (574) 535-7599 from a cell phone.
- If there is no immediate danger to the health or safety of yourself or others, but you believe there is an ongoing or potential hazard that may need attention, contact Campus Security through Omnilert or (574) 535-7599 from a cell phone.
