Driving Application

*****Please be sure to read the Vehicle Policy

Employees must complete a new driver’s online application form and the MVR form. Please attach a copy of your driver’s license at the bottom of the form.

***In addition, an MVR (motor vehicle report) will be conducted prior to an employee or student being granted driving privileges.  Please print and complete the MVR Release Consent Form and send/bring it to the physical plant office.

Students must complete a new application form when driving for a department(s).

Fill out the Paper Driver’s Application (.doc), have it signed by your supervisor, and drop it off at the Physical Plant along with a copy of your license.
